Приложение на проксимална комплементна блокада в лечението на пароксизмална нощна хемоглобинурия: терапевтична ефективност и безопасност на iptacopan
Abstract
Пароксизмалната нощна хемоглобинурия (ПНХ) е рядко клонално немалигнено хематологично заболяване, което се характеризира с хронична комплемент-медиирана интраваскуларна хемолиза. Поради развитието на ятрогенна екстраваскуларна хемолиза при лечение с С5 инхибитори възниква необходимостта от разработване на проксимални инхибитори на комплемента. За разлика от С5 инхибиторите, тези агенти блокират комплемента в етапи, предшестващи активирането на С5, и по този начин потискат механизмите както на интра-, така и на екстраваскуларна хемолиза при ПНХ. Iptacopan е перорален инхибитор на фактор В от алтернативния път на комплемента и регулира разцепването на С3.
В настоящия обзор са представени данни от клинични изпитвания относно ефективността и безопасността на iptacopan монотерапия за лечение на ПНХ.
